This notice explains how information submitted through this booking page is handled. It applies to the Free 30-Minute Longevity Introduction and does not replace any privacy notice provided when you become a patient.

Information collected

When you book, you may provide contact and scheduling information such as your name, email address, telephone number, time zone, and appointment preferences.

Please do not submit medical information—including symptoms, diagnoses, treatment details, medications, insurance information, or emergency requests—through the booking form. The introduction is not medical care or an emergency service.

How information is used

Information may be used to schedule and conduct the introduction, send appointment communications, protect the booking service, respond to requests, and measure completed bookings from advertising.

Service providers

The booking experience may use Calendly, Microsoft Outlook, Zoom, Meta advertising tools, and the website hosting provider. These providers process information under their own terms and privacy practices.

Retention and choices

Information is retained only as reasonably needed for scheduling, communications, security, recordkeeping, and applicable legal obligations. You may request access, correction, or deletion, subject to legal and operational requirements.
